wanting to refer students to REACH for educational support services
should follow the procedures below:
Calgary
Board of Education (CBE)
1. Consult with a CBE Student Services strategist/specialist to determine if the student should be referred to REACH.
2. Determine the service(s) required.
- If you are unsure which services are appropriate, please contact REACH for more information.
3. Contact the student’s parent(s) or legal guardian(s).
- Determine whether other agencies are currently involved with the student (e.g. Regional Health Authority)
- Have the parents/guardians complete the Home Information (Form B1) and Authorization for Release of Information (Form B2).
- If a Psycho-educational assessment is being requested, the parents must complete the Parental Authorization for Psychological Assessment
4. Gather all of the pertinent information needed:
- Medical reports
- Previous assessment documents
- Relevant information from school file.
5. Complete the Referral for REACH Services: School Information (Form A)
- Attach all required documentation
- Ensure an Audiogram is attached if requesting support from an Educational Audiologist or Hearing Consultant.
- Ensure an Ophthalmology report is attached if requesting support from a Vision Consultant.
6. Submit all completed forms to Special Education, Parkdale.
7. Once REACH receives the Referral form, a Status of Referral form will be faxed to the school/jurisdiction (Parkdale) indicating the following:
- The referral is complete; student assigned for service (disciplines specified)
- The referral is waitlisted
- The referral is incomplete. The required information will be specified.
- The student is not eligible for service based on documentation received.
Other
School Jurisdictions and Private Schools
1. Determine that the student meets REACH eligibility criteria.
2. Determine the service(s) required.
- If you are unsure which services are appropriate, please contact REACH for more information.

- Determine whether other agencies are currently involved with the student (e.g. Regional Health Authority.)

7. Once the Referral form has been received, one of two things will happen:
(a) A REACH Services Agreement will be sent to the school or jurisdiction. the Service Agreement outlines th eservice that will be provided.
(b) A Status of Referral form will be faxed to the school and/or jurisdiction indiciating the following:
- The referral is waitlisted
- The referral is incomplete. The required information will be specified.
- The student is not eligible for service based on documentation received.

Note: The
Authorization for Release of Information and ACH Consent to Release
Information need to be mailed to REACH, as original signatures are required
by outside agencies.
The School
is responsible for distributing, collecting and submitting all forms
that complete the REACH Referral Package.
